Positivity rates for staining (>= grade 1) ranged from 41.3% to 84.1% (mean = 0.8 +- 0.9 grade), TBUT (< 10s) varied from 39.1% to 61.6% (mean = 10.4 +- 6.6 seconds), tear osmolarity (> 308 mOsm/L) ranged from 63.7% to 72.4% (mean = 319.7 +- 20.8), MGD grading ranged from 28.9% to 51.3% (mean = 0.5 +- 0.7), and OSDI symptom scores ranged from 57.6% to 71.0% (mean = 23.5 +- 20.5) across sites. Among these markers, tear osmolarity showed the greatest consistency between sites (max/min positivity = 114%), followed by OSDI (123%), TBUT (158%), MGD (178%), and staining (204%). DED markers showed little correlation with each other (average r2 = 0.05 +- 0.07). Notably, a significant number of participants (N = 110) had high symptom scores (OSDI = 32.4 +- 15.7) and hyperosmolarity (338.1 +- 20.1 mOsm/L), but exhibited no other clear signs of DED (MGD grade = 0.2 +- 0.4, TBUT = 13.5 +- 7.0 seconds, staining grade = 0.4 +- 0.5). In conclusion, subjective assessments of DED showed considerable variation, whereas objective measurements like OSDI and tear osmolarity demonstrated greater consistency across sites. Many participants presented with significant symptoms and hyperosmolarity without other overt signs of DED, often remaining undiagnosed without osmolarity testing.
